Items listed below are examples of required information needed to determine residence, household, resources and income. More information may be required.

Driver’s license, state identification or birth certificate
Copy of last month’s paycheck stubs or letter from employer stating your gross pay
Social Security Disability Insurance (SSDI)
Supplemental Security Income award letter (SSI)
Payments from pension plan
Value of all vehicles
Vehicle insurance card
Any health insurance cards
Other income sources
Social Security card/number for all household members
Proof of Matagorda County residency
Child support income
Food Stamp status letter
Workers compensation statement
Last bank statement

Unemployed Applicants
Must show registration with the Texas Work Commission, must apply for unemployment benefits and if disabled, must apply for Disability Benefits.

I-MAP and L-MAP participants are required to obtain care from the Public Health Clinic, except in an emergency.
