Public Health Clinic

Applicant must provide all information and documentation which is requested and the application must be completely filled out, signed and dated. Verification of income, residence, household composition and resources are required. Copies of all requested documentation must be submitted with application.

We will process your application within 14 days of when all the required information is received. We will determine your eligibility for one of the programs and notify you either in person or by mail. Applicants have the right to appeal denials.

Residence
An eligible applicant must be a citizen who resides in Matagorda County and must intend to remain here.

Household
A household is a person living alone or two or more persons living together where legal responsibility for support exists, excluding disqualified persons. A disqualified person is one who receives or is eligible to receive Medicaid.

Resources
A household is eligible if the total countable household resources do not exceed $3,000 when a person who is aged or disabled and who meets relationship requirements lives in the home or $2,000 for all other households.

I-MAP Income (Indigent)
An I-MAP household is eligible if its monthly net income does not exceed 21% of Federal Poverty Guidelines (FPG).

L-MAP Income (Low Income)
An L-MAP household is eligible if its monthly net income is between 21% & 70% of Federal Poverty Guidelines (FPG).

Over Income
Applicants who do not meet the requirements will be referred to MEHOP and/or a local physician.

Items listed below are examples of required information needed to determine residence, household, resources and income. More information may be required.

  • Driver’s license, state identification or birth certificate
  • Copy of last month’s paycheck stubs or letter from employer stating your gross pay
  • Social Security Disability Insurance (SSDI)
  • Supplemental Security Income award letter (SSI)
  • Payments from pension plan
  • Value of all vehicles
  • Vehicle insurance card
  • Any health insurance cards
  • Other income sources
  • Social Security card/number for all household members
  • Proof of Matagorda County residency
  • Child support income
  • Food Stamp status letter
  • Workers compensation statement
  • Last bank statement

Unemployed Applicants
Must show registration with the Texas Work Commission Must apply for unemployment benefits If disabled, must apply for Disability Benefits
